Community Partner And Resources
Instructions
Collaborate with community members, community liaisons, and/or members of health care team regarding your project. Identify these community partners and collect information regarding the assets and resources of your selected community, and how. Relate how they will be helpful in implementing your project. (These don’t have to be large organizations, but can be people instrumental to your project.)
This assignment will include:

3. Describe their roles in your project.
