the interview for this discussion involves a 35-year-old male with a history of morbid obesity with disabilities in a rural setting. Communication and interview techniques for this patient would differ from other patients based on the patient’s gender, young age, rural setting, and comorbidities. Being a mid-30-year-old male in a rural setting myself would give me common ground to relate to the patient and build rapport. Targeting questions based on the patient’s economic status might involve whether his disabilities leave him unable to work. Questions involving his education could be targeted by the rural setting and either poor access to or lack of finishing school for work or service-related reasons.
When targeting inquiries based on the patient’s economic condition, one question that may come up is whether or not the patient’s limitations prevent him from working. It is possible that questions about his schooling will be asked because he was raised in a rural area and either had limited access to formal education or did not complete it for professional or humanitarian reasons. Questions revolving around the rural environment, physical distance to healthcare, and how the patient’s weight affects transportation and healthcare access could be used to tackle the issue of healthcare access. Healthcare access could be targeted by questions revolving around the rural setting, physical distance to healthcare, and how the patient’s obesity affects transportation and healthcare access. Targeting questions on neighborhood and built environment could involve physical distance to food, exercise, or assistance and lack of developed areas for healthy activities. Social and community context questions would involve the patient’s beliefs and cultural attitudes towards healthcare, disability, and obesity, as well as food and lifestyle habits.
Health risks for this patient are many since morbid obesity is a risk factor for many illnesses and diseases. Major health risks include diabetes mellites (Type II), sleep apnea, depression, and many cardiovascular diseases. Chronic illnesses may be less pertinent to the patient’s age; preventing their development will be especially important. The CDC (2022) states that rural Americans are more likely to die prematurely from heart disease, cancer, unintentional injury, chronic lower respiratory disease, and stroke than their urban counterparts. What specific disabilities the patient suffers from will be important in determining how the other factors affect the patient’s risks.
A risk assessment instrument for identifying cardiovascular disease would be most appropriate for this patient. Early identification of cardiovascular disease could help prevent heart disease and stroke. Questions regarding this would revolve around symptoms such as tiredness, inability to perform tasks, shortness of breath, swelling, and more. Questions about physical activity, habits such as smoking, and diet would be important. Tests that would benefit in screening for cardiovascular risks would include blood pressure, cholesterol, and blood sugar (AHA, 2019).
Five targeted questions to ask this patient could be:
1. “What concerns do you have that brought you to seek medical care today?”
2. “Do you consider yourself to be overweight or unhealthy?” (This is a less open-ended question that directs the assessment toward the patients’ beliefs)
3. “Do you have any family members who have or have died from heart disease?” (Alternately: “Tell me about some of the health problems in your family.”)
4. “How often do you see a healthcare professional, and how difficult is it for you to seek medical care?”
5. “Are there any activities of daily living that are more difficult for you than they used to be?”
