1. Identify a planning model that could be used to help determine the needs of the community and provide justification for your selection. (10 points)

4. Physical exercise – how often do you exercise?
5. Lifestyle and food – what types of foods and drinks do you consume?
6. Symptoms such as fatigue, dizziness, paleness, and poor vision. Are you experiencing feelings of fatigue, dizziness, paleness, and poor vision?

b. Conduct a readability test of your survey by using the readability statistics in Microsoft Word (see directions that follow).
1. Click File tab, then Options.
2. Click proofing.
3. Under When correcting spelling & grammar in Word, be sure the Check grammar with spelling is selected by checking the box.
4. Select the Show readability statistics by checking the box.
5. Then, when the document is complete, click spell check. Once spelling and grammar check is complete, you should see a box regarding readability (see reading level).

c. Do you think your survey is at a reading level that is appropriate for those who will complete the survey? Justify your answer. (2.5 points)
The reading level is appropriate for the target audience. It has simple words, standard words per sentence, and characters per word. The sentences per paragraph make it easier to read and understand the concepts. I also used additional evaluation to ensure the responses are matching with the problems affecting the target audience.
