1. Identify a planning model that could be used to help determine the needs of the community and provide justification for your selection. (10 points)

The selected planning model is a written community survey. The purpose of selecting the community survey is due to the benefit of attracting more responses. It is unlike the use of telephone interviews or face-to-face interviews. Telephone or email survey has low returns which cannot assist in assessing the needs in the community. Communities have varying levels of literacy which requires utilization of simple methods of collecting information.

2. How does the planning model that you selected relate to the Generalized Model? (10 points)

Generalized model and a written community survey are effective strategies effective for collecting reliable information to assess the needs of a population. A needs assessment would be effective to identify the needs such as health factors, environmental factors, social issues, or family involvement. Generalized model is effective in analyzing complex issues in the community. The community survey will provide additional insights into the problems that individuals cannot express by word of mouth.

3. What quantitative data collection method would you use to obtain information from your service population?
The best quantitative data collection method is survey methodology.

a. What method will you use and why? (10 points)

The written survey method is effective since it attracts higher returns from the target population. For example, telephone surveys may not be effective since some access to remote areas will be a challenge. A face-to-face interview will make individuals uncomfortable if the interview is focusing on sensitive issues such as reproductive health. A written survey will be effective since it can be used to gather comprehensive data about the status of the community. A record is necessary to assist the professionals who will analyze the data to keep a record and analyze the data effectively.

b. What type of information is needed? (5 points)

The information required includes the patients demographics such as age, gender, and occupation, health status, the risk factors, and the social and physical environment. The respondents should fill the information in the survey materials without identifying their identity. Confidentiality will help the respondents to open up and provide reliable and authentic information without fear of intimidation.

c. From whom is the information needed? (5 points)
The information will collected from the sample size. The sample size will be a representative group of the entire target population. It will be important to ensure that the sample size is representative in terms of number, gender, and age. Confidentiality will be observed in the collection of data.

4. Suppose you will use a survey to collect information about your disease of choice.

a. Prepare a list of 6-8 questions that you would ask your service population about your disease of choice, along with the appropriate response categories. Note: You may only have one open-ended question. The rest should have categories of response (i.e. a 1-5 rating scale, yes/no, ABCD, etc.) Be sure to read CDC Evaluation Brief # 15 for the checklist to evaluate the quality of questions. (15 points)

Risk factors for diabetes mellitus
The open-ended questions include;
1. Weight – what is your weight, height, and BMI?
2. Medical history – what are remarkable moments in your medical history?
3. Hereditary factor – do you have family members with diabetes?
4. Physical exercise – how often do you exercise?
5. Lifestyle and food – what types of foods and drinks do you consume?
6. Symptoms such as fatigue, dizziness, paleness, and poor vision. Are you experiencing feelings of fatigue, dizziness, paleness, and poor vision?

b. Conduct a readability test of your survey by using the readability statistics in Microsoft Word (see directions that follow).
1. Click File tab, then Options.
2. Click proofing.
3. Under When correcting spelling & grammar in Word, be sure the Check grammar with spelling is selected by checking the box.
4. Select the Show readability statistics by checking the box.
5. Then, when the document is complete, click spell check. Once spelling and grammar check is complete, you should see a box regarding readability (see reading level).
Record the readability statistics. (5 points)

c. Do you think your survey is at a reading level that is appropriate for those who will complete the survey? Justify your answer. (2.5 points)
The reading level is appropriate for the target audience. It has simple words, standard words per sentence, and characters per word. The sentences per paragraph make it easier to read and understand the concepts. I also used additional evaluation to ensure the responses are matching with the problems affecting the target audience. The wording and presentation is simple and direct-to-the-point to enhance clarity.
d. If your answer is “no” to the previous question, revise your survey as necessary and report the changes you made here. (2.5 points)
The answer is yes and no need to revise the content.
