Abstract
To address the health disparity associated with infant mortality rates, there is a need for an advanced health care innovation to improve maternal health care services in the society. In most states, Lavin, Harper and Barr (2015) argues that more black babies are likely die at a higher rates than white babies. The prevalence of infant mortality is high among the minority communities and thus, it is important to implement effective health care innovations to improve maternal health care. Some of the reasons for the health disparity in the provision of quality maternal health care includes the differences in access to health care and the education about maternal health services. For example, a health education app is an innovation that can trigger improved maternal health care services. As a result, the paper evaluates the process of defining the grant proposal to understand the implementation of new health care innovation.

Grant Proposal
Introduction
Addressing infant mortality disparity can be a severe challenge without having health care innovation. Some of the reasons of the increased rate of health disparity in infant mortality includes the lack of supportive regulations, absence of ethical standards and provisions, and the limited research on maternal health care services. For example, weak and old maternal health system and restrictive regulations in maternal health care services have contributed to the higher disparity in infant mortality rates. A recommendable health care innovation is creating and implementing a health education app for the pregnant mothers (Fleiszer, et al., 2015). However, the health innovation would only effective, if accessible to all mothers irrespective of their race and background. Such an innovation would work well in reducing health disparity in infant mortality rates. The health care innovation would be effectively and useful in addressing the health care disparity in maternal health. There is need for the health care innovation to assist in promoting faster development of the treatment actions and plans (Lavin et al., 2015).The use of technology would assist in customizing the maternal health programs in an effective way to reduce infant mortality disparity rates. The new health care innovation would also help in increasing the overall access to care. Therefore, disparity in infant mortality can be resolved through investing in new health care innovation to improve access to care services and increase the faster development of treatment options.
Intended Goals of Health Care Innovation
The implementation of health care innovation will be linked to clear intended goals including;
1. To reduce maternal health infections among minorities by 50% in the next 3 years. The health education app will create awareness about the potential health infections during pregnancy. Such efforts will assist in reducing the level of maternal health infections.
2. To lower rates of preventable complications by 15 percent in the 12 months. Infant mortality disparity can be reduced through eliminating cases of preventable complications.
3. To redesign the health delivery process to improve maternal safety by 30% in 6 months. The health education app will also educate the health care professionals on the relevant safety standards that should be followed to improve efficiency and effectiveness of health care delivery.
4. To improve medication safety by 20% in the next one year through documentation and innovative medication system. It involves ability of the health care professionals to recommend the correct medications for the pregnant mothers. The health education app comprises information for health professionals and the pregnant mothers making it possible to diagnose conditions and allocate proper drugs.
List Budget
Health education apps for pregnant mothers is a good technology and innovation that can be used in increasing awareness to improve maternal health care. The health innovation app can be implemented through steps and processes to reduce health disparity in infant mortality rates.

Most importantly, the health innovation will improve the potential to achieve long-term health goals through the implementation of effective evidence-based programs such as coaching. Such project would also inspire prevention of health disparities in maternal health as well as expanding the reach to the minority communities. It will be possible to improve health services offered to the minority communities through giving them more maternal health information and awareness on causes of infant mortality.
According to Gardner, Gardner and O’connell (2014), the nursing discipline is focused on the overall transformation of health care and showing their commitment to change. Nurses are usually committed to develop new plans and adopt new health innovations that promotes health care services across the various spectrums. Nurse-led approaches would improve the maternal care programs to lower the hospitalization rates in the minority communities. It would improve access to care through creating awareness that enhances coordination between the health care professionals and the patients. Therefore, the implementation of new health innovation will be effective in enhancing the accessibility, sustainability, and the efficiency of maternal health systems to combat infant mortality disparity.
Nursing contribution to innovation is critical and widely recognized in the society. The input of nursing in innovation helps to support the progress of health care system in the health care facilities. Nurses operates in environment with the varied people including patients, families, personnel, and the communities. The nursing discipline is well-positioned to offer creative and innovative programs to make a significant impact (Lavin, et al., 2015). The health education app will provide the nurses with comprehensive information and diagnosis records to assist in understanding infant mortality disparity in the communities. It will be an innovative solution to help in reducing the struggle for equitable and safe maternal health in the communities as the nurses will be able to reach to all sidelined communities. The health care innovation might be technical, involving new administrative structures. However, the contribution of the nurses remains vital in improving the performance of the new innovation in reducing infant health care disparity in the society.
Increased access to health care programs would help the minority communities to get access to quality health care services. Health care innovation is also needed to lower costs and prevent potential errors during the provision of maternal health services. The nurses will help in the sustaining of the health innovation and ensuring that it works on improving quality maternal care. For example, the nurses will be able to get data and approaches of how to improve on the innovation to create quality maternal health for all people irrespective of their ethnic and background differences (Gardner, et al., 2014). The health education apps relies on the input of the nursing practitioners to be able to work effectively in improving maternal health care.
Conclusion
In summary, infant mortality disparity rates requires the implementation of a health care innovation to reduce the gap between the minority communities and wealthier groups in the access to quality maternal health care. The goals are to ensure that the health care innovation improves medication safety, enhances reduction of infections, dealing with preventable complications, and allowing for the redesign of the health care delivery programs. The considered health care innovation is the implementation of a health education app for expectant mothers. The health education app will be able to improve awareness on potential complications related with pregnancy. However, the health care innovation has a significant nursing implication because the input of the nurses is vital. The nurses understands the health disparity effectively and their coordination with the various stakeholders will assist in reducing disparity in infant mortality rates. As a result, the nurses should work with the technical professionals to implement the health care innovation effectively.
