The nursing diagnosis indicates that the increased level of disturbed sensory perception of uncontrolled diabetes leads to the slow hearing foot wound.

The objectives of the learning instructions include that the short term goal was to ensure that the foot infection is gone and sensory perception will be back in two weeks. The patient will need to ambulate without help. The potential intervention would is to teach the patient about compensating for the loss of the sensory perception through ensuring that the foot wound is healing well. It is also recommendable to keep the bed lowered while offering assistance to dealing with the sensory perception. Lowering the bed helps Tina in moving without putting more pressure on the foot wound. Other interventions include the implementation of a self-check skin program. The patient will need to be informed about the need to evaluate the integrity of the skin, disturbed sensory perception, and edema conditions.
Regarding evaluation, the patient will need to show their knowledge on the skin checks and how to conduct them accordingly. The patient will have the right and complete sensory perception in the right foot before the discharge. The patient would not also be injured because of the reduction in the sensory perception. The long-term goal is to maintain the regular and stable glucose levels for the entire year without additional skin infections. It is important to improve the nutrition and medication while treating diabetes. Some of the interventions critical to the long-term progress include setting a physical and medical goals, teaching how to conduct medication as well as it is necessary to schedule regular meetings with the nutritionist. The nutritionist would offer the additional information about the potential risks associated with diabetes. Based on Jarvis (2015), the major aspects in recommending medications for Tina includes the recording oft the medical diagnosis, the assessment of findings, the nursing diagnosis, setting goals, interventions and then evaluations. Following the discharge plan, it will help in improving the efficiency of the treatment plan to improve the speed of discharging the patients which are useful.
