The proposed database will be used to compile and report on the clinical data related to quality measures. The change of technology and specifically the increase in the use of electronic health records system has led to the emergence of blackened electronic data repositories of patient. Therefore due to the growth of the records, there is need for creation of a database for quality control and analysis. More information is collected in hospital on a daily basis and therefore the collected information should be used for direct patient care improvement (Chelico et al., 2016). ln most cases it is easy to store data in a database and perform several analyses but it is no easy to use the resources for quality improvement. The proposed project is designed for improvement of the quality of services.
Definitions · Database: A large collection of data that is stored and organized in a computer for easy retrieval for other uses, expanded and updated. · Data Network: A set of databases that are connected by several computers and serve a large number of users.
· Demographic Data: Consists of data that provides information on age, gender, race, marital status, address of residence, and other information about patients · Administrative Data: Facts concerning health insurance for instance benefit package, eligibility, and membership among others · Unique personal identifies: Are applied only to a specific person, and the information does not change over time.
The database users will be the staff of different hospitals, the quality control team, and the stakeholders. The personnel, for instance, will be the director of medical services who will be given almost all the rights to access the database. The assistance medical doctors whose rights to access the database will be almost equivalent to the director of medical services. The quality control team will only have to access data that is connected to quality requirements. The workers and other stakeholders will be granted limited access to the system. The credentials are given depending on the function of each individual.
lnformation that should be generated includes the views of people that will be collected through interviews or observation method. The information that must be compiled includes the details on the demographics of patients (age, sex, and address), diagnosis of patients, procedure, admission source, discharge status, possible length of stay and the charges (Muji et al., 2009). The information will also be collected from publication and the records available in different hospitals. Different reports will be generated from the database. Diagnostic reports will be used to monitor the progress of patients, the report on drug utilization and patient’s response report to assess any possible effects and the availability of drugs (AHRQ.gov, 2019). Other reports include demographic and staff reports to monitor both the staff and the patients The goals include: to improve the accuracy of the data related to patients and distributors of drugs, to increase the safety of administration of medication and to reduce any possible risks of care-associated infections.
The objective is to design an integrated system for ensuring quality of the patients’ care or the utilization of pharmaceutical system in hospitals. The goals and objectives affect the data models in terms of design.

The steps for the design of the database will follow the waterfall model. The process will include establishing the requirements, data requirements, data analysis, conceptual data model, implementation and testing.
Future barriers would be the security of the database and specialized skills for maintaining and operating the database. There could be the possibility of interference of data that may compromise the quality of data (Schneeweiss, 2010).
The database will enhance the quality of service delivery and administration of drugs. The information stored in the database will help in epidemiological studies and analysis for provision of quality healthcare. The information will also help in the administrative planning for improvement of management of healthcare.
