Application: Health Care Data Dictionary
The IBM Dictionary of Computing defines a data dictionary as a “centralized repository of information about data such as meaning, relationships to other data, origin, usage, and format.” (IBM, 1993) It is related to descriptions pertaining to databases and database management systems (DBMS).
The American Health Information Management Association (AHIMA) defines a data dictionary as a descriptive list of names (also called representations or displays), definitions, and attributes of data elements to be collected in an information system or database. (AHIMA, 2006). The purpose of the data dictionary is to standardize definitions and ensure consistency of use.
To prepare for this Application, read the following in your course text Health Information: Management of a Strategic Resource:

The Database Programmer
For this Application, select a disease state of your choosing. In a 1- to 2-page report written in Help write my thesis – APA style:
Determine at least 10 fields of significance for a database pertinent to the disease state selected. Be sure to include data fields such as a patient’s PHI, address, insurance information, and fields particular to the disease state chosen.
Describe the importance to all stakeholders within the clinical, technical, and management aspects of health care delivery to have data entry forms be constructed using a well-conceived and standardized data dictionary.
Explain how standardized forms deliver value to the integration between HIS Applications running locally and remotely.
Comment upon the complexity of integration when multiple HIS Applications do not follow similar data dictionaries.
