Week 7: Healthcare Equity and Access in Specialty Areas
Access to healthcare has been an ongoing primary concern of public health officials, political leaders, and the general public. As specialty areas continue to emerge and evolve, concerns over healthcare equity and access only increase.
As problem solvers adept at applying critical thinking and reasoning, clinical specialty nurses can be difference makers where healthcare disparities exist. Nurses are uniquely positioned to contribute to and lead transformative change in healthcare settings. They can be contributors to interprofessional teams driving changes or addressing the needs of patients who are impacted by such change.
This means that nurse educators must help provide enhanced knowledge, skills, and attitudes that are agile and change with the healthcare environment. Nurses must be prepared to further employ tools such as data analytics, quality improvement, and more in the quest to manage and even drive change to better reach appropriate populations.
For this week, you will examine disparities in healthcare as they relate to equity and access.

The focus of your second entry in your journal will be on quality in your clinical specialty. This week, you will complete and finalize this journal entry.
Your journal entries represent a reflection of the experiences and observations you have made during your practicum. You must demonstrate how you are advancing your knowledge in your clinical specialty area.
To prepare and develop your journal entry:
• During your practicum time, as you note experiences and anecdotes that are instructive for you as an advanced nurse, consider how you might use those examples as an advanced nurse.
• Reflect on how you can integrate clinical expertise in your work as an advanced nurse educator.
• As you develop the journal entry outlined below, keep in mind that your goal is to provide a narrative account that captures your clinical experiences and demonstrates how you are advancing and applying your knowledge in the area of quality as it applies to advanced physical assessment, pathophysiology and pharmacology.
• Review the QSEN Competencies (https://qsen.org/competencies/pre-licensure-ksas/) related to quality as you prepare for your journal entry.
Journal Entry 2 (450–500 words)
• Describe a selected, observed activity or situation. (Note: This should not be more than half a page) I’m completing my practicum experience at a Women’s Health Clinic ( Obstetrics and Gynecology) I observe women receiving annual physical exams and pap smears to check for abnormal cells in the uterus that could produce cancer. There are also breast exams performed and women are educated on the importance of breast exam.
• Explain what you observed in the context of quality indicators in your specialty of interest. Use the Learning Resources and/or the best available evidence from current literature to support your explanation. An example quality indicator is how reminders are set to alert the provider and other medical staff that the patient is due for screening. The patient is then notified to schedule appointment for exam. Cervical Cancer Screening is very important. The interdisciplinary team works together to make sure the screening happens and if something is abnormal the team works together to make sure that proper care, referrals, and appointments are scheduled to manage care. It takes the entire team to do their part. The team consists of a Physician, Nurse Practitioner, Medical assistants, nurses, lab, ultrasound techs, office managers, and office staff.
