The Problem
Elderly patients in the City of Grapevine experience health issues that require an effective response. One of the best responses is the provision of nutritional food to boost immunity and improve recovery (Fernández-Barrés et al., 2017). Statistics indicate that the City of Grapevine has a rising number of elderly people suffering from chronic diseases, mental health, and malnutrition (Michalczyk et al., 2020). Nutrition can play a major role in improving the quality of health in the elderly populace in the City of Grapevine.
Intervention
Educating elderly patients on how to eat, exercise and live a healthy life is essential. The programs can help the patients to enhance the treatment of various chronic diseases or mental disorders (Tripicchio et al., 2019). For instance, nutritional education is essential to help elderly patients to prevent developing health conditions such as diabetes. The intervention should focus on what to eat, the amount, the sources of nutrients, and available supplements (Dave et al., 2017). It is also essential to address issues related to obesity and how elderly patients should cope up. Nutritional education also involved informing the patients on the need to refrain from drug abuse, alcohol, and prevention of infectious diseases.
Quality Initiative
One of the best quality initiatives is pharmacist-led medication therapy management. It focuses on reducing the cost of care per patient, minimize hospital admissions, and decrease medication count (Choi et al., 2016). The quality initiative is essential to reduce cases of hospitalization among elderly patients. For example, elderly patients are seeking health services to address cardiovascular problems or mental disorders (Desveaux et al., 2019). Significance and Implications
The City of Grapevine is home to thousands of elderly people who are affected by at least one disease or are at risk of infection. Healthcare providers can participate in educating the population about the importance of nutrition (Miyazaki et al., 2017). The educational intervention will reduce the rate of infection, the number of affected people, the number of deaths, and enhance patient recovery. The intervention will also prevent other health cases in the future.
Proposed Solution
The proposed solution is to use various channels to educate the population of elderly patients. Education initiatives are appropriate in hospitals to patients who are seeking care. It is also essential to create awareness in public to promote better public health outcomes (Michalczyk et al., 2020). The awareness will be essential to eliminate myths and misinformation about nutrition. Education channels may include digital methods, brochures, and fliers. The solution should realize better healthcare among the elderly by preventing diseases and improving the rate of recovery.
